Talk the visit. Sign the SOAP.
Doctors record a voice note in the Medichive app describing the consult. Google Cloud Speech-to-Text transcribes it; the AI drafts the SOAP note plus history deltas. The doctor reviews, edits and signs — usually in under a minute.
Doctors spend more time on paperwork than patients
Physicians spend nearly half their time on documentation. The conversation that mattered already happened — typing it out twice slows everyone down and pulls attention away from the room.
Traditional method
- 30–60 minutes of typing after each session
- Errors and omissions from memory recall
- Unstructured prose, no codes
- Hours-to-days before the chart updates
Medichive voice → SOAP
- 30-second in-app voice note per visit
- SOAP draft on your dashboard in <1 min
- Structured S/O/A/P, ICD-11-ready
- Chart updates the instant you sign
The actual capture-to-chart pipeline
Six things this system does well — each maps 1:1 to code running today in the Medichive backend.
In-app voice capture
Open the patient in the Medichive web or Android app and tap record. Capture a single note or stitch a few together over the visit. Audio uploads straight to encrypted Cloud Storage and the transcription pipeline runs asynchronously.
Medical-grade STT
Google Cloud Speech-to-Text with the medical_conversation model, speaker diarization and 16 kHz audio — tuned for clinical vocabulary out of the box.
Structured SOAP drafting
The AI, with tool-use output, produces a Subjective / Objective / Assessment / Plan note as structured JSON — never freeform text. Prompt caching keeps the per-note cost down.
Auto-merge history deltas
A second AI pass extracts what's new — vitals, diagnoses, meds, allergies, referrals, imaging orders — and merges it into the patient's longitudinal timeline. No copy-paste between systems.
Signed & immutable
Signing computes a SHA-256 hash of the note and writes a tamper-evident JSON snapshot to encrypted Cloud Storage. Edits after signing go through addenda, not overwrites.
ICD-11 analyse, on demand
Search any ICD-11 code on a patient's chart and the AI synthesises matched findings across history, reports and uploaded documents into one summary — against whatever's been signed.
From voice note to signed chart
Three steps, two minutes of doctor time. Most of the heavy lifting happens between steps 1 and 2 while the doctor walks to the next room.
Send the voice note
Open the patient in the Medichive app (web or Android) and tap record. Capture one or more voice notes describing the visit, then tap done when finished.
AI drafts the note
Cloud STT transcribes the audio; the AI extracts a Subjective / Objective / Assessment / Plan structure with diagnoses, meds and follow-ups. The draft appears on your dashboard ready for review.
Review, edit, sign
Open the draft, edit anything that needs correcting, hit sign. The note becomes immutable, the SHA-256 hash is stored, and the longitudinal record updates with new diagnoses, meds and orders.
Where it fits today
Outpatient consults
20-patient days where each note takes 5 minutes to type. Capture once in the app, sign 20 notes from your laptop between sessions.
Tele-consults
Drop a voice note immediately after the call ends. The SOAP draft is ready before the next patient connects.
Specialty follow-ups
Cardiology, endocrinology, pulmonology — recurring patients where most of the value is the longitudinal record. Wearable vitals already live alongside the note.
Home visits
No laptop in the bag — just the Medichive Android app on your phone. The chart updates when you sign from anywhere with internet.
